Our enquiry and visit forms ask for a name, phone number, email address, a preferred date or general care category, and an optional message. The submitted details are prepared in the visitor’s own email application and addressed to Agham.

What not to send through public forms

Please do not include medical histories, medication lists, diagnoses, national identity/passport numbers, financial details or other sensitive personal information in a public enquiry. Agham’s care team can advise on a suitable private process if further information is needed.
